2024 marked a significant milestone as Joanna celebrated 20 years of clinical practice in Traditional Chinese Medicine (TCM). After several years of working with pregnant women through TCM and massage, Joanna attended a seminar in 2008 led by Debra Betts (https://acupuncture.rhizome.net.nz/), one of the leading experts in modern pregnancy acupuncture. It was at this seminar that Joanna felt a strong sense of purpose and knew that this was the path she wanted to pursue for her career.

Since then, Joanna has participated in nearly every professional Western lecture on TCM for pregnancy and postpartum care, both in-person and online, to enhance her knowledge and clinical skills. This dedication has enabled her to treat hundreds of women and families throughout her practice.

From 2022 to 2024, Joanna attended the Maternity Acupuncture Mentorship Program (MAMPs https://www.mamps.org/), further enriching her education and clinical experience. As a member of the Obstetric Acupuncture Association (OBAA https://ob-acupuncture.com/), she stays current with the latest research in her field.

From the beginnings…..

Acupuncture and Traditional Chinese Medicine (TCM) have been utilized in pregnancy for centuries, and there is extensive documentation of their practices. Three of the oldest texts from classical China include:

1. 'Taichanshu' - 'Book on the Fetus and Gestation' (163BCE)

2. 'Qianjinfang' (652 C.E.), which was translated from 'Bei ji qian jin yao fang' - 'Essential Formulas for Emergencies Worth a Thousand in Gold,' translated by Sabine Wilms (https://www.happygoatproductions.com/).

3. 'Ishimpo' - 'Prescriptions from the Heart of Medicine' (982 C.E.).

These texts discuss proper maternal behaviours, dietary recommendations and prohibitions, herbal prescriptions for pregnancy-related conditions, and channel theory, providing month-by-month guidelines for usage or restrictions throughout the pregnancy.

To modern day…… Acupuncture Professional Guidelines & Research

In today's world, pregnancy acupuncture is regarded as a safe complementary treatment alongside standard obstetric care and is practised in many hospitals worldwide, including China, Germany and the USA. A growing body of research consistently reinforces the safety of acupuncture during pregnancy, which should be very reassuring for mothers seeking this type of care.

Top obstetric colleges worldwide recognise acupuncture as an effective treatment for various pregnancy and birth conditions. The American College of Obstetricians and Gynecologists (ACOG) suggests that acupuncture has the ‘potential to assist in labour progression during labour dystocia’, which involves incorrect birth positioning. While ACOG found ‘no significant impact on cesarean delivery rates, there is evidence indicating that acupuncture may enhance patient satisfaction with these interventions’(1).

In the UK, the Royal College of Obstetricians and Gynaecologists (RCOG) recommends non-pharmacological interventions, such as acupuncture and massage, as first-line treatments for pregnancy-related headaches, lower back pain, and pelvic pain before considering analgesic options(2). In their ‘Information for You - Patient Handout - June 2015’, they include acupuncture as a treatment option for pelvic girdle pain in pregnancy(3).

The Royal Australian and New Zealand College of Obstetrics and Gynaecologists (RANZCOG), in their 2016 Patient Handout titled "Pain Relief in Labour & Childbirth," notes that natural pain relief options like acupuncture, acupressure, touch, and massage can be effective but should only be administered by qualified practitioners(4).

Additional professional documents, such as "The Australian Pregnancy Care Guidelines," recommend acupuncture as a treatment for pelvic girdle pain during pregnancy (5). Furthermore, the Royal College of Midwives (UK) discusses the potential benefits of acupuncture and moxibustion for addressing breech presentation(6).

Cochrane systematic reviews have demonstrated positive effects of various Traditional Chinese Medicine (TCM) modalities for different pregnancy conditions. These include:

  • Moxibustion for breech presentation (1)

  • Acupuncture for pain management during labour (2)

  • Acupuncture or acupressure for labour induction (3)

  • Acupuncture for treating low back and pelvic pain during pregnancy (4)

  • Acupuncture that improves sleep quality and appetite during pregnancy (5)

  • Acupuncture as an adjunct to usual care for depression (6)

  • Acupoint massage to prevent mastitis after childbirth (7)

These findings highlight the potential benefits of TCM in addressing a wide range of pregnancy-related issues.

In a search of the research in the last five years, numerous randomised controlled trials and systematic reviews have been conducted in this area. While many of these studies are small and further research is needed, it is encouraging to note that positive results have been found in various fields of study, including:

  • Safety of acupuncture during pregnancy (1)

  • Acupressure for pelvic and lower back pain in pregnancy (2)

  • Acupuncture for non-serious headaches (3)

  • Acupuncture, acupressure, and auriculotherapy (ear acupuncture) for nausea, vomiting, and hyperemesis gravidarum in pregnancy (4)

  • Acupuncture, acupressure, and auriculotherapy for labour pain, labour progress, and induction of labour (5)

  • Acupuncture, acupressure, and moxibustion for breech presentation (6)

  • Auriculotherapy for improving sleep quality (7)

  • Acupuncture for depression during pregnancy (8)

Many studies are also being undertaken for the postpartum period, which is great. Some of the topics being investigated include:

  • Acupuncture and acupressure for breast tenderness and lactation issues, including low milk volume. (1)

  • Acupuncture as an adjunct therapy for postpartum pelvic floor issues, including stress incontinence (2)

  • Moxibustion for uterine involution (3)

  • Acupuncture combined with exercise for diastasis recti (4)

  • Acupuncture and auriculotherapy for pain following a cesarean section (5)

  • Acupuncture for postpartum depression (6)

  • Acupuncture and cupping for postpartum back pain (7)

This research highlights the potential benefits of acupuncture and related practices during and after pregnancy.
